Position Description:

This position is located at one or more of our clinical sites in the Bronx, Brooklyn, or Queens.

NURSE PRACTITIONERS (report to the collaborating physician at the contracted facility) shall:

  • Reach out and engage clients with appropriate concern for their health and other needs;
  • obtain health histories, perform physical examinations and screening tests including testing for HIV on site;
  • Assess and treat common and acute and chronic illnesses including HIV/AIDS (in accordance with
  • Approved protocols) and refer clients when necessary to provider's facility or other appropriate facility and follow up such referrals;
  • Conduct health screening services;
  • Make every effort to provide and assure on‑going medical service including a continuum of care for persons with HIV/AIDS;
  • Advise and counsel clients about their physical and mental health;
  • Provide consultation and training to on‑site staff and volunteers where appropriate;
  • Assist with the identification and counseling of mental health and substance abuse problems;
  • Work in a non-traditional environment; e.g., non-clinic facility where homeless people congregate.

  • New York State Nurse Practitioner License
  • Commitment to the poor and disadvantaged

While performing the duties of this job, the employee is regularly required to talk or hear. The employee regularly is required to stand, walk, sit, and to use their hand to finger, handle or feel objects, tools or controls; and reach with hands and arms. The employee frequently lifts patients, stretches and has other physical exertion during patient visits and while moving equipment. Specific vision abilities required by this job include close vision and the ability to adjust focus.

While performing the duties of this position, the employee travels regularly by automobile, subway or bus and is exposed to changing weather conditions. The employee is required to travel to meetings with clients and/or other individuals and to outreach sites. The employee may have to travel and spend time over night in another locale.
